Assistance with life stage transition service includes short and long terms support that aims at strengthening and supporting the participant’s abilities and choices. Transitions such as moving out of one’s home, joining the university, or getting a job are part of everyone’s life. It is never an easy task both emotionally and practically, and it becomes a little more difficult for people with disabilities. Change can be hard and with it comes new environments and settings. Under this NDIS support category, Sensible Care offers a wide range of supports to assist life stage transitions. The idea is to make this transition phase as smooth as possible without disrupting the comfort of the participant. We work with the participant to enable a friction-free move and encourage the participant to settle in a new setting and create a comfortable attitude. Some our services for this purpose include:
